About 10.50am a white Toyota Yaris was travelling east on Shanley Road in Jarrahdale.
At the same time a green Toyota Corolla was travelling north on the South Western Highway.
The vehicles collided at the intersection of the two roads.
The driver of the Toyota Yaris, a 92-year-old man from Port Kennedy, was airlifted to Royal Perth Hospital by the RAC Rescue Helicopter, but he later died.
A male passenger in his vehicle was also injured, and taken to Armadale Hospital.
The driver of the Toyota Corolla, a 33-year-old woman, was taken to Armadale Hospital with minor injuries.
