Police allege Midland man Bradley John McIntosh-Narrier (27) threatened guards with a metal rail he had pulled from the wall of a hospital toilet before making a getaway through the campus roof about 1.50pm on January 17.
It is alleged he stole a Toyota Rav 4 from a woman outside JHC and was on the run until Sunday afternoon when officers detained him in Armadale.
The Rav 4 was found in Balcatta on Saturday.
Police charged him with 29 offences after he allegedly committed a number of crimes while evading capture, including armed robbery, vehicle theft and burglary.
Regional Investigations Unit detectives allege the accused was attempting to steal a Toyota Camry from the driveway of a home when he was arrested.
He was under police guard in Royal Perth Hospital yesterday receiving treatment for injuries suffered in a car crash prior to his arrest.
