Victorian police had charged the 31-year-old at the time, but he allegedly failed to appear in a Victorian court when required in August, 2012.
Senior Sergeant Brad Burnett said he had since moved to WA.
On May 15, WA Police arrested the alleged offender who appeared in Perth Magistrates Court on May 18, where his extradition was approved.
About four years ago, it is alleged the man was part of a group that targeted occupants of a home in the Melbourne suburb of Heidelberg West.
It is alleged the men armed themselves with weapons, including hockey sticks, tomahawks and axe handles.
One of them allegedly attacked a man at the home with an axe handle, but Sen Sgt Burnett said the group retreated when they realised they were outnumbered.
It is alleged they smashed all the windows at the home.
One of the men allegedly returned later to threaten the occupants with a sawn-off shotgun. The local man was charged with causing injury, reckless injury, affray and criminal damage.
